2016--2019年广州市农村9〜12岁儿童青少年 血压偏高与体质指数的关联分析.

Objective To investigate the relationship between high blood pressure and body mass index (BMI) in children and adolescents, to provide reference for effective prevention and control measures of high blood pressure. Methods The data were collected from National Surveillance of Common Diseases and Health risk factors among students in Guangzhou from 2016 to 2018 and from the Nutrition Improvement Project of Guangzhou Rural Compulsory Education Students in 2019. In total 2 021 children and adolescents aged 9 to 12 years old were selected. Multivariate logistic regression was used to analyze the relationship between high blood pressure and BML The percentage of population attributable risk was calculated and the effect of obesity control on the prevention of high blood pressure was evaluated. Results The detection rate of high blood pressure among children and adolescents aged 9 to 12 years in rural areas of Guangzhou from 2016 to 2019 was 5.7%, and the detection rates of high simple systolic blood pressure, simple diastolic blood pressure, and mixed blood pressure were 3.2%, 3.3%, and 0.8%, respectively. Blood pressure increased with age. The detection rate of high blood pressure in public school students was higher than that in private school students (於=16.556, P<0.001), and that in non-resident school students was higher than that in resident school students (於=13.421, P<0.001)» With the increase of BMI (=23.106, P<0»05) the detection rate of blood pressure in public school students was higher than that in non-resident school students. The detection rate of high blood pressure in boys and girls in the obesity group was 2.7 times and 3.8 times higher than that in the normal group, respectively. Obesity was a risk factor for high blood pressure 95%CI: 1.8-5.1). The percentage of attributable risk of obesity to people with high blood pressure was 153%, After obesity was controlled, the expected detection rate of high blood pressure decreased to 4.8%. Conclusion The high blood pressure of 9-12-year-old children and adolescents in rural areas of Guangzhou is mainly caused by simple diastolic blood pressure, and the blood pressure increases gradually with the increase of BML The prevention and control of obesity is the key measure to prevent high blood pressure. 目的探讨儿童青少年血压偏高与体质指数(BMI)之间的关系,为制定有效的血压偏高防控措施提供依据。方法 数据来源于2016-2018年"全国学生常见病及健康危险因素监测"工作中广州地区数据及2019年"广州市农村义务 教育学生营养改善项目"数据, 选取资料完整的9 ~ 12岁儿童青少年2 021名。采用於检验进行组间率的比较,采用多 因素logistic回归分析血压偏高与BMI之间的关系,计算人群归因危险度百分比并评估控制肥胖后对预防血压偏高的 影响。结果2016--2019年广州市农村9 ~ 12岁儿童青少年血压偏高检出率为5.7%,其中单纯性收缩压偏高、单纯性 舒张压偏高和混合性血压偏高检出率分别为3.2%、3.3%和0.8%。血压随年龄逐渐增高, 血压偏高检出率呈现为公办学 校学生高于民办学校学生(於=16.556,Pv0.001),非住校学生高于住校学生(泻13.421,PvO.OOl),随BMI的增大而逐 渐增加(於趋势=23.106,P<0.05)o肥胖组男生和女生血压偏高检出率分别为正常组的2.7倍和3.8倍, 肥胖是血压偏高 的危险因素(Od3.1,95%CZ:1.8~5.1)。肥胖对血压偏高的人群归因危险度百分比为15.3%,控制肥胖后, 血压偏高的 预期检岀率降至4.8%。结论 广州市农村9~12岁儿童青少年血压偏高以单纯性舒张压偏高为主, 血压随BMI的增大 而逐渐增加,预防和控制肥胖是预防血压偏高的关键措施.
