A 13-year-old girl with B-cell acute lymphoblastic leukemia developed hypersensitivity and serum sickness reactions while being treated with calaspargase-pegol. After receiving the second dose of calaspargase-pegol, she experienced symptoms such as vomiting, itching, chest tightness, and hives. The medication was stopped, and her symptoms improved with the use of ondansetron. However, she later developed erythema, pain, and swelling in her thumb, followed by joint pain in her wrists, hands, feet, ankles, and knees. This was determined to be a delayed serum sickness reaction, and she was treated with celecoxib and prednisone, which resolved her symptoms. She continued to receive treatment with crisantaspase.
