The European multicenter Concorde trial of AZT involving 1749 people followed over a three-year period found no difference in time progression of AIDS or survival in the AZT-treated versus placebo group. On tuberculosis (TB), HIV-induced immunosuppression is the highest risk factor for activation of latent infection and disease. The disease then progresses faster in HIV-seropositive individuals and is more severe and difficult to treat than in people not infected with HIV. Non-compliance with treatment is resulting in a rise in multiple drug resistant TB. With six major subtypes of HIV-1 identified, it will be several years before a safe and effective vaccine against HIV may be available. The risk of HIV-1 transmission from mother to child is reported to range from 15-20% in Europe, 25-39% in Africa, and 16-30% in the US; the risk of HIV-2 transmission, however, is less than 5%. It appears that increased risk of transmission is associated with advanced maternal disease status, p-24 antigenaemia, premature delivery, and breastfeeding, yet no single factor is predictive. Evidence indicates that women who become infected with HIV during the time they are breastfeeding are twice as likely to transmit the virus to theirs infant than women who were infected before beginning breastfeeding. Finally, STDs increase the risk of HIV transmission, but the causative mechanisms are poorly understood. STD prevention and control measures should be taken accordingly.
