Melanoma primario cerebral. (Spanish)

Statistically, intracranial malignant melanoma in the population is approximately 0.005 cases / 100 000, with a prevalence towards the male population. Principles have been described to the approach for diagnosis and surgical and oncologic treatments. We present a case report of a man 39-year-old, with evolution characterized by headache, lateralization gait, dysmetria, dysdiadochokinesia, truncal ataxia and Romberg. Starting the diagnostic protocol is identified by MRI the presence of a lesion in the right posterior fossa, with significant edema. That is why it is scheduled for surgical management, achieving total resection of the lesion and subsequent report by the Clinical Pathology Service as melanoma. Immunohistochemistry tests being positive S100 and HMB 45 in the extension studies for ophthalmology, dermatology and gastroenterology with conducting Panendoscopy was not possible to find another source of tumor growth. The study of positron emission tomography showed no positivity across the previously known save intracranial growth. Conclusion: patterns of diagnosis of melanoma of the central nervous system make it essential to thoroughly investigate possible sites of origin and rule out metastatic lesions;regarding the management, consider total ablation wherever possible and the use of aggregate cancer therapy. Estadísticamente los melanomas malignos intracraneales en la población es de aproximadamente 0.005 casos por 100,000 habitantes, con una prevalencia hacia la población masculina. Se han descrito principios para el abordaje diagnóstico; así como, para el tratamiento quirúrgico y adyuvante oncológico. Reporte de caso: masculino de 39 años de edad, con evolución caracterizada por cefalea, lateralización de la marcha, dismetría, disdiadococinesia derechas, ataxia troncal y Romberg. Iniciando el protocolo diagnóstico se identifica por resonancia magnética la presencia de una lesión en fosa posterior de mayor afección cerebelosa derecha, heterogénea, con edema importante. Por lo cual es programado para manejo quirúrgico, logrando resección total de la lesión, con reporte posterior del servicio de patología clínica como melanoma. Se realizan pruebas de inmunohistoquímica siendo positivas S100 y HMB 45. En los estudios de extensión por oftalmología, dermatología y gastroenterología con realización de panendoscopía no fue posible localizar otro foco de crecimiento tumoral. El estudio de tomografía por emisión de positrones no demostró positividad en otro lado salvo el crecimiento intracraneal previamente conocido. Conclusión: las pautas del diagnóstico de melanoma de sistema nervioso central hacen indispensable el investigar exhaustivamente posibles sitios de origen y descartar lesiones metastásicas; respecto al manejo, se debe considerar la ablación total siempre que sea posible; así como, el uso de terapia oncológica agregada.
