Managementul durerii severe la pacienții cu cancer - analiză a literaturii de specialitate 2017-2020. (Romanian)

Objective: The aim of the paper was to review the literature to understand the scientific use of strong opioidsforthe management of severe background pain and incidents in cancer patients. Material and method: A systematic literature review was performed: 42 articles were included published in the period 2017-2020. Results: Morphine (po), Fentanyl (transdermal) and Oxycodone (po) were the first three most commonly administered opioids. In the case of severe background pain, the oral administration of opioids was mainly used, and for incident pain, the oral and transdermal route. The side effects of opioids are respiratory disorders due to apnoea and the risk of infections, which increases by 2% for every 10 mg of oral morphine. In the case of cancer of the abdominal-pelvic organs, the main trigger of pain was food ingestion, and in the case of lung cancer food ingestion, movement and cough. The combinations agonist + antagonist, Oxycodone-Naloxone and Morphine-Naloxone have a good analgesic effect and reduce the risk of side effects and toxicity by adding the antagonist. The effectiveness of Methadone is similar to that of Fentanyl, and Morphine and Oxycodone administered orally have an analgesic effect similar to that of Fentanyl and Buprenorphine administered transdermally. The main reasons for opioid rotation were: inefficient analgesia, side effects and dysphagia. Methadone can be used successfully in oxycodone-refractory pain. Two new modes of opioid conversion were found. Conclusion. The treatment of severe pain in patients with neoplasms remains a challenge for the medical system. Grade III opioids, especially Morphine, Fentanyl and Oxycodone are the pharmaceutical methods of choice for combating pain in these patients. [ABSTRACT FROM AUTHOR]
Obiectiv: Scopul lucrării a fost să revizuiască literatura de specialitate pentru a înțelege modul ştiințific de utilizare a opioidelor puternice pentru managementul durerii severe de fond şi incidente la pacienții cu cancer. Material şi metodă: O revizuire sistematică de literatură; s-au inclus în studiu 42 de articole publicate în perioada 2017-2020. Rezultate: Morfina (per os: po), Fentanylul (transdermal) şi Oxicodona (po) au fost primele trei opioide administrate cel mai frecvent. În cazul durerii severe de fond s-a folosit preponderent administrarea orală a opioidelor, iar pentru durerea incidentă calea orală dar şi transdermală. Efectele secundare ale opioidelor sunt tulburările respiratorii datorită apnee şi riscul de infecții, acesta crescând cu 2% pentru fiecare 10 mg de Morfină orală. În cazul cancerului organelor abdomino-pelvine, principalul factor declanşator al durerii a fost ingestia de alimente, iar în cazul cancerului pulmonar ingestia de alimente, mişcarea şi tusea. Combinațiile agonist+antagonist, Oxicodonă-Naloxonă şi Morfină-Naloxonă prezintă un efect analgezic bun şi scad riscul de efecte adverse şi de toxicitate prin adăugarea antagonistului. Eficiența Metadonei este similară cu a Fentanylului, iar Morfina şi Oxicodona administrate oral au efect antialgic similar cu a Fentanylului şi Buprenorfinei administrate transdermal. Motivele principale de rotație a opioidelor au fost: analgezia ineficientă, efectele adverse şi disfagia. Metadona poate fi utilizată cu succes în cazul durerii refractare la Oxicodonă. S-au constatat două moduri noi de conversie la schimbarea opioidelor. Concluzie: Tratamentul durerii severe la pacienții cu neoplazii rămâne încă o provocare pentru sistemul medical. Opioidele de treaptă a III-a, în special Morfina, Fentanylul şi Oxicodona reprezintă metodă farmaceutică de elecție pentru combaterea durerii la aceşti pacienți. [ABSTRACT FROM AUTHOR]
