IDIOPATHIC OROFACIAL PAIN The general dentist is likely to be the first health care professional to face the challenge of diagnosing patients with idiopathic orofacial conditions. These conditions are relatively rare, and their etiology and pathophysiology are not fully understood. The International Classification of Orofacial Pain (ICOP) defines four idiopathic orofacial pain diagnoses: burning mouth syndrome, persistent idiopathic facial pain, persistent idiopathic dentoalveolar pain, and persistent unilateral facial pain with additional attacks. The first step in the management of these conditions is a thorough clinical examination to exclude other causative lesions and ICOP diagnoses. An adequate knowledge of these conditions and a systematic diagnostic process are essential for avoiding unnecessary invasive and irreversible treatments that may exacerbate symptoms. Treatment is symptomatic and can be topical or systemic medication, or behavioral approaches. The role of the general dentist varies according to the condition, clinician knowledge, and experience, but an initial examination and exclusion of local causative pathology should be done before the patient is referred to a specialist. Den alment praktiserende tandlæge er sandsynligvis den første sundhedsperson, der står over for udfordringen med at diagnosticere patienter med idiopatiske orofaciale smertetilstande. Disse lidelser er relativt sjældne, og deres ætiologi og patofysiologi er ikke fuldt ud forstået. Den internationale klassificering af orofacial smerte (ICOP) definerer fire idiopatiske orofaciale smertediagnoser: burning mouth syndrome, persisterende idiopatisk ansigtssmerte, persisterende idiopatisk dentoalveolær smerte og konstant unilateral ansigtssmerte med ekstra anfald (CUFPA). Det første trin i håndteringen af disse tilstande er en grundig klinisk undersøgelse for at kunne udelukke andre forårsagende læsioner og ICOP-diagnoser. En tilstrækkelig viden om disse tilstande og en systematisk diagnostisk proces er afgørende for at undgå unødvendige invasive og irreversible behandlinger, der kan forværre symptomerne. Behandlingen er symptomatisk og kan være lokal eller systemisk medicin eller adfærdsmæssige tiltag. Den alment praktiserende tandlæges rolle varierer afhængigt af tilstanden samt klinikerens viden og erfaring; men en indledende undersøgelse og udelukkelse af lokal årsagssygdom bør foretages, før patienten henvises til en specialist.
