Impacto de la transfusión de concentrados de hematíes en pacientes críticos de una UCI polivalente de la Argentina. (Italian)

Objective: To assess if red blood cell transfusion in the critically ill patient is associated with a higher mortality rate in the ICU. Design: Retrospective cohort study in patients >18 years hospitalized at least 24 h in a polyvalent ICU from July 2009 to December 2018. Variables of interest: Number of red blood cell transfusions and ICU mortality. Results: 4244 patients were analyzed (male 64.6%) with an average age of 50.35 (± 17.14) years, average days of hospitalization in ICU 6.61 (± 8.94), APACHE II 14.74 (± 8.67), SAPS II 36.87 (± 19.40), SOFA 4.96 (± 3.72), 46.9% required mechanical ventilation, mortality rate in ICU 24.6%. 735 patients required red blood cell transfusions (17.3%). When they were compared with those who had not received a transfusion, the former were younger (51 vs. 53 years, p <0.001), had higher severity scores (APACHE II 17 vs. 13 p <0.001; SAPS II 40 vs. 32 p <0.001; SOFA 6 vs. 4 p <0.001), longer ICU stay (6 vs. 3 days, p <0.001), increased need of mechanical ventilation (65.7 vs. 42.9%, p <0.001) and mortality rate (34 vs. 22.6%, <0.001). The multivariate logistic regression analysis showed that the patients with the highest number of red blood cell transfusion had higher mortality rate in the ICU [odds ratio 1.066 (95%CI 1.015-1.119; p 0.01)], adjusting for age, days of hospitalization, severity scales, and need for mechanical ventilation. Objetivo: Evaluar si la transfusión de concentrados de hematíes en el paciente crítico se asocia a una mayor tasa de mortalidad en la UCI. Diseño: Estudio de cohorte retrospectivo en pacientes >18 años internados, al menos, 24 h en una UCI polivalente, entre julio de 2009 y diciembre de 2018. Variables de interés: Cantidad de transfusiones de concentrados de hematíes y muerte en la UCI. Resultados: Se analizaron 4244 pacientes (hombres 64,6%) con una edad promedio de 50,35 (± 17,14) años, días de internación 6,61 (± 8,94), APACHE II 14,74 (± 8,67), SAPS II 36,87 (± 19,40), SOFA 4,96 (± 3,72), necesidad de ventilación mecánica 46,9% y tasa de mortalidad en la UCI 24,6%. El 17,3% requirió transfusiones de concentrados de hematíes. Cuando se los comparó con los no transfundidos, los primeros tenían menos edad (51 vs. 53 años, p <0,001), puntajes de gravedad más altos (APACHE II 17 vs. 13, p <0,001; SAPS II 40 vs. 32, p <0,001; SOFA 6 vs. 4, p <0,001), estancia más prolongada en la UCI (6 vs. 3 días, p <0,001), más necesidad de ventilación mecánica (65,7 vs. 42,9%, p <0,001) y tasa de mortalidad más elevada (34 vs. 22,6%, p <0,001). El análisis de regresión logística multivariable mostró que los pacientes con más cantidad de concentrados de hematíes transfundidos tuvieron una tasa de mortalidad más alta en la UCI [odds ratio 1,066 (IC95% 1,015-1,119; p 0,01)], ajustando por edad, días de internación, escalas de gravedad y necesidad de ventilación mecánica.
