Systemic capillary leak syndrome (SCLS) is a rare, severe illness with a high mortality rate, during which are observed acute episodes of hypotension, haemoconcentration and hypoproteinemia with three characteristic stages. It is due to an increased capillary permeability to proteins. There are two types of SCLS - idiopathic, also known as Clarkson’s disease, found more often in adults, with recurrent course and a detectible monoclonal protein in the serum of majority of patients, and secondary - a complication of different diseases, most often viral infections, as well as a drug induced. The pathophysiology involves hyper inflammatory response alongside with endothelial genetic defect (currently proven only in test animals) with different cytokine profile in children and in adults. The survival rate without proper treatment is very low, so recognizing the disease is critically important. During the now passing COVID-19 pandemic there have been case reports of idiopathic and secondary SCLS, related to both the SARS-CoV-2 infection and the administration of vaccines. As far as we know only one has been in a child. We present the case of a 3 month old infant, in whom during the course of hospital treatment of COVID-19, presenting with gastrointestinal symptoms and a secondary urinary infection we observed periphery edema, coinciding with oliguria which led to a period of anuria, hypotension, increasing weight, hypoproteinemia, including hypofibrinogenemia, haemostatic disturbances and haemoconcentration. With the appropriate tests other diseases with similar symptoms and laboratory constellations were excluded and SCLS was diagnosed. The patient received therapy consisting of judicious dosed intravenous rehydration with crystalloids and substitution with plasma products, which had a good clinical effect and led to a normalization of the laboratory markers. The follow up in the next 12 months show normal growth and development and no other episodes of edema. [ABSTRACT FROM AUTHOR]
