Splenectomies in Newfoundland and Labrador: A 10-year evaluation.

Splenectomy may be performed for many reasons and represents an important diagnostic and therapeutic surgical procedure. We retrospectively examined the histology of splenic neoplasms in Newfoundland and Labrador over a ten-year period to assess the frequency and indications for splenectomy procedures throughout the province. A total of 176 splenectomies were performed. The top three indications for splenectomy were surgery for malignancy (48, 27%), trauma (33, 19%) and idiopathic thrombocytopenic purpura (24, 14%). Vascular tumours accounted for 17 cases and lymphoma was identified in 23 cases. The three most common lymphomas included splenic marginal zone lymphoma, diffuse large B cell lymphoma, and hairy cell leukemia. The fifth edition of the World Health Organization Classification of Hematolymphoid Tumors was recently published, which reflects an updated classification of primary splenic tumours and lymphomas. We emphasize the need to reclassify previously diagnosed splenic lymphomas to incorporate these updates, with the goal of enhancing patient management and prognostication. [ABSTRACT FROM AUTHOR]
Il existe de nombreuses raisons de procéder à une splénectomie, qui représente une intervention chirurgicale diagnostique et thérapeutique importante. Nous avons examiné de manière rétrospective les caractéristiques histologiques de tumeurs spléniques à Terre-Neuve-et-Labrador sur une période de dix ans afin d'évaluer la fréquence et les indications des splénectomies dans l'ensemble de la province. Au total, 176 splénectomies ont été effectuées. Les trois principales indications motivant cette intervention étaient la présence d'une tumeur maligne (48, 27 %), d'un traumatisme (33, 19 %) et d'un purpura thrombocytopénique idiopathique (24, 14 %). Les tumeurs vasculaires représentaient 17 cas et des lymphomes ont été diagnostiqués dans 23 cas. Les trois lymphomes les plus fréquents étaient le lymphome de la zone marginale splénique, le lymphome diffus à grandes cellules B et la leucémie à tricholeucocytes. La cinquième édition de la classification des tumeurs hématolymphoïdes de l'Organisation mondiale de la Santé a récemment été publiée et reflète une mise à jour de la classification des tumeurs spléniques primitives et des lymphomes. Nous insistons sur la nécessité de reclasser les lymphomes spléniques précédemment diagnostiqués afin d'intégrer ces mises à jour, dans le but d'améliorer la prise en charge des patient·e·s et le pronostic.
