Effektive Glykolyse-Inhibierung im Citrat-gepufferten venösen Vollblut und Plasma/Effective inhibition of glycolysis in venous whole blood and plasma samples. (German)

Background: To stabilise glucose immediately and to prevent preanalytical loss of glucose from blood samples after collection, additional acidification is needed. Methods: In this study, we aimed to assess the value of citrate-buffered GlucoEXACT tubes (Sarstedt) compared to VenoSafe™ Glycaemia (Terumo) tubes. Conventional sodium fluoride (NaF) tubes were used as internal controls. First, stability of venous plasma glucose concentrations were compared in 10 healthy volunteers at nine definite time points (after 0 - 0,5 - 1 - 1,5 - 2 - 3 - 4 - 8 and 12 h). Blood samples from VenoSafe™ Glycaemia and NaF tubes were immediately centrifuged and separated. Moreover, glucose concentrations in an additional five healthy volunteers were compared by collecting blood samples simultaneously in GlucoEXACT and VenoSafe™ Glycaemia tubes and storing for 0 - 1 - 2 - 4 - 12 - 24 and 48 h until centrifugation. Dilution- and haematocrit-dependent bias of plasma glucose in GlucoEXACT tubes was assessed by serial dilution. Results: Mean Terumo plasma glucose concentrations decreased within the first 4 h from 94,9 mg/dL to 92,5 mg/dL (-2,5%) and remained stable after 12 h. Mean conventional NaF plasma glucose concentrations decreased within the first 4 h from 90 mg/dL to 87 mg/dL (-3,3%, not significant) and decreased significantly after 12 h (82,6 mg/dL, -8,2%). There was no significant decrease or difference of plasma glucose levels in stored blood samples collected in GlucoEXACT and VenoSafe™ Glycaemia tubes [linear regression analysis: R 2 =0,9688; GlucoEXACT (y)=5,3871+0,9431×VenoSafe™ Glycaemia (x); Wilcoxon test, p=0,7125]. Citrate buffer in GlucoEXACT tubes does not impair glucose concentration. Conclusions: Determination of plasma glucose concentration in GlucoEXACT tubes is a suitable diagnostic alternative compared with Terumo VenoSafe™ Glycaemia tubes. [ABSTRACT FROM AUTHOR]
Einleitung: Komplette und sofortige Inhibierung der Glykolyse im venösen Vollblut proximal der Enolase-Hemmung ist durch Natrium-Fluorid (NaF)-Additiv nicht möglich, so dass zusätzliches Ansäuern die effektive Hemmung des Glukoseabbaus ermöglicht. Methodik: Wirkung der Glykolyse-Inhibierung in Citrat-gepufferten GlucoEXACT-Röhrchen (Additiv: 0,4 mL, Sarstedt) wurde gegen VenoSafe™ Glycaemia-Röhrchen (Additiv: Pulvergemisch, Terumo; Empfehlung DDG/DGGG-Leitlinie Gestationsdiabetes) verglichen. Parallel wurden konventionelle NaF-Röhrchen (Sarstedt) verwendet. Bei 10 gesunden Probanden wurde die Stabilität der Glukosekonzentration nach 0 - 0,5 - 1 - 1,5 - 2 - 3 - 4 - 8 und 12 Stunden Lagerung von sofort zentrifugierten und von den Zellen getrennten venösen (VenoSafe™ Glycaemia)- und (NaF)-Plasmaproben untersucht. Bei weiteren 5 gesunden Probanden wurden Citrat-gepufferte venöse Vollblutproben in VenoSafe™ Glycaemia- bzw. GlucoEXACT-Röhrchen entnommen und nach 0 - 1 - 2 - 4 - 12 - 24 und 48 Stunden Lagerung die Plasmaglukosekonzentrationen bestimmt. Die Verdünnungs- und Hämatokrit-Abhängigkeit des Sarstedt-Röhrchens wurde in einer NaCl-Verdünnungsreihe sowie durch den Zusatz von EDTA-Zellvolumen untersucht. Ergebnisse: Sofort getrennte Terumo-Plasmaglukosekonzentrationen zeigten in den ersten 4 Stunden einen Abfall von 94,9 mg/dL auf 92,5 mg/dL (-2,5%) und blieben nach 12 Stunden stabil. NaF-Plasmaglukosekonzentrationen verringerten sich von 90 mg/dL auf 87 mg/dL (-3,3%; nicht signifikant), um nach 12 h deutlich auf 82,6 mg/dL (-8,2%) abzufallen. Gelagerte Vollblutproben in GlucoEXACT- und VenoSafe™ Glycaemia-Röhrchen wiesen keine signifikant unterschiedlichen Plasmaglukosekonzentrationen bei 0-4 bzw. 12-48 Stunden auf (lineare Regression: R 2 =0,9688; GlucoEXACT (y)=5,3871+0,9431× VenoSafe™ Glycaemia (x); Wilcoxon-Test: p=0,7125; gute Übereinstimmung der Einzelglukosekonzentrationen: Kappa-Koeffizient 0,702; 95% CI: 0,616-0,783). Sowohl das Volumen des Additivs in den GlucoEXACT- als auch das Pulvergemisch in den Terumo-Röhrchen haben keinen Einfluss auf die Glukosekonzentration. Schlussfolgerung: GlucoEXACT-Röhrchen können als diagnostisch gleichwertige Alternative zur Bestimmung der Plasmaglukosekonzentration verwendet werden. [ABSTRACT FROM AUTHOR]
