Pazienti ad alta e bassa complessità clinica: impatto sul carico di lavoro infermieristico. (Italian)

Introduction: Patients admitted to the intensive care unit greatly differ in severity and intensity of care. Material and methods: retrospective observational study. Patients enrolled in a general intensive care unit, where the score of nursing activities was carried out daily, were enrolled. For each enrolled patient, the application of the classification devised by Iapichino and collaborators in 2006 was applied, to identify the patients "High level of care - HLC" and those "Low level of care - LLC". Results: 187 patients were enrolled in the period April 2010-March 2013. The median age of the sample was 62 years (IQR: 47-75 - Range 3-86) with a value of 17 for the median of the hospital stay (IQR: 6-28 - range 5-128). The median NAS during ICU stay was equal to 73 (IQR: 67-80, range 39-158), while the median of SAPS 2 had a value equal to 41 (IQR: 31 - 52, - Range 6-99). The median of the NAS in LLC days was equal to 68 (IQR 55-75, range 39-110) while in HLC days the median of the NAS was equal to 81 (IQR 49- 94, range 42-155). The difference in the NAS value in the two classes was not statistically significant (P = 0.075). The correlation between NAS and the LLC / HLC subdivision (Spearman correlation) was r= .213 (p <0.001). Conclusions: The use of the LLC / HLC classification system not correlate clinical severity and nursing complexity. The daily use of the NAS can help nursing coordinators organize nurses daily presence in relation to the number of beds available based on the complexity of care. [ABSTRACT FROM AUTHOR]
Introduzione: La grande eterogenicità dei pazienti ricoverati in terapia intensiva ha amplificato la diseguaglianza tra gravità clinica e complessità assistenziale. In passato sono stati proposti in terapia intensiva dei sistemi che, basandosi sul numero di insufficienze d'organo presenti nei pazienti, permettessero di identificare i pazienti a bassa versus alta complessità. Materiale e metodi : studio osservazionale retrospettivo. Sono stati arruolati i pazienti ricoverati in una terapia intensiva generale, in cui il Nursing Activities Score veniva rilevato giornalmente. Per ogni paziente arruolato, è stata successivamente applicata la classificazione ideata da Iapichino e collaboratori nel 2006, per identificare i pazienti "High level of care" e quelli "Low Level". Risultati: Nel periodo Aprile 2010-Marzo 2013 sono stati arruolati187 pazienti. La mediana dell'età del campione è stata pari a 62 anni (IQR : 47- 75 -- Range 3- 86) con una valore di 17 per quanto riguarda la mediana della degenza (IQR : 6-28 -- range 5-128). La mediana del NAS dell'intero ricovero, dei pazienti inclusi nello studio è stata pari a 73 (IQR : 67-80, range 39-158), mentre la mediana del SAPS 2 ha avuto un valore pari a 41 (IQR: 31-52, -- Range 6-99). La mediana del NAS nelle giornate LLC è stata pari a 68 (IQR 55-75, range 39-110) mentre nelle giornate HLC la mediana del NAS è stata pari a 81 (IQR 49-94, range 42-155). Le differenze del valore NAS nelle due classi non è risultata essere statisticamente significativa (P=0.075). La correlazione tra il NAS e la suddivisione LLC/HLC (Spearman's correlation) è stata pari a r=.213 (p<0.001). Conclusioni: L'utilizzo del solo sistema di classificazione LLC/HLC non permette di correlare gravità clinica e complessità assistenziale. L'utilizzo quotidiano del NAS può aiutare i coordinatori infermieristici a programmare le presenze giornaliere in relazione al numero di letti disponibili ed in base alla complessità assistenziale.
